I request to be admitted as a member of the Public Organization "UNION OF FOOTBALL PLAYERS OF TAJIKISTAN". I undertake to comply with the Charter and the Membership Regulations of the Union, to regularly pay membership dues in the manner and amount determined by the Union, to comply with the decisions of the governing bodies of the Public Organization, and to participate in the activities of the Public Organization to the extent possible.

In accordance with the Labor Code of the Republic of Tajikistan and Law No. 1537 dated August 3, 2018, "On Personal Data Protection", and for the purpose of protecting my personal rights and professional interests, I give my consent to the processing of my personal data for the entire period of my membership in the Public Organization "Union of Football Players of Tajikistan".

I have familiarized myself with the Charter and the Membership Regulations of the Public Organization "Union of Football Players of Tajikistan".
